One useful book that I have not yet seen mentioned is Elaine G. Breslaw's "Lotions, Potions, Pills, and Magic: Health Care in Early America" (2012). It's a short, accessible synthesis of work done on medicine and health in early America, and it also contains a helpful bibliographic essay. A bit tougher to find but also very useful is "Medicine in Colonial Massachusetts, 1620-1820" (1980) published by the Colonial Society of Massachusetts.
