I do not have an appliance, but I use Appriver ( http://www.appriver.com/ ). I am very happy with them. They filter spam and viruses. All of our mail goes to them first to be filtered and then it is sent to my mail server. If my mail server or Internet connection is down, they automatically spool it mail until it is up. They also host Exchange mail accounts if you want. I figure that the cost of an appliance becomes affordable around 160 mail accounts or more, if you amortize the cost of the appliance over 5 years. I have 155 mail accounts now and I do not have to deal with another machine. They have a web interface to access the filtering software. You can set up individual accounts for people to manage their own spam.
