If you are looking for a solution that is local, allows annotation, and is also free you might want to look at Wink (http://www.debugmode.com/wink/). I haven't used it in a while but as I recall it allow narration and text notes; you can also use it to create clickable buttons that take the user to different parts of the project. It outputs as a Flash file.
